What is the IMM 5644 form?

IMM 5644 pdf is a document checklist that you need to use for renewing your Canadian Permanent Resident Card.  Permanent Resident Card is proof of status that permanent residents of Canada need to carry with them at all times. What steps should I take in case if I have lost my card?

I have lost my permanent resident card outside of Canada

You need to report about your stoled PR card to the nearest Canadian Embassy or Canadian immigration office. You have to obtain a travel document before your new PR card is issued. In this case, you need to prove your identity and permanent resident status in Canada. Later, after returning to Canada, you can apply for a new permanent residence card in the country.
