IMM 5644 is e document checklist that you need to use for renewing your Canadian Permanent Resident Card. Permanent Resident Card is proof of status that permanent residents of Canada need to carry with them at all times. - A photocopy of the main identification document
- You will need photocopies of two additional forms of identification. For example, you can provide a driver’s license, an identification card from the province where you reside or other available official documentation.
- You must provide eligible and complete photocopies of Income Tax documents, school transcripts or other available documents in order to prove that you meet residency requirements.
- Also, you need to submit two photos of you that were taken in the last six months before you applied. In addition, please note that you should avid of using paperclips or staples with the photos.

Will I lose my status if I forget to renew my card?
No. You will not lose your permanent resident status if you have not renewed your permanent resident card.
What steps should I take in case if I have lost my card?
If you lost permanent resident card Canada, you must immediately contact Immigration, Refugees and Citizenship Canada either by calling them (1-888-242-2100) or visiting their nearest office.
I have lost my permanent resident card outside of Canada
You need to report about your stoled PR card to the nearest Canadian Embassy or Canadian immigration office. You have to obtain a travel document before your new PR card is issued. In this case, you need to prove your identity and permanent resident status in Canada. Later, after returning to Canada, you can apply for a new permanent residence card in the country.
